The Role
What to expect as a Support Worker with On Track Care
Key Responsibilities
- Provide person-centred support aligned with participants' NDIS plans and goals
- Assist with daily living activities including personal care, meal preparation, and household tasks
- Support community access activities and social participation
- Maintain accurate documentation and progress notes
- Communicate effectively with participants, families, and the On Track Care team
- Follow all workplace health and safety policies and procedures
- Respect participant privacy and confidentiality at all times
- Participate in team meetings and training sessions as required
Requirements
- Current NDIS Worker Screening Check (or willingness to obtain)
- Current Working with Children Check (Blue Card) (or willingness to obtain)
- Current First Aid and CPR certification (or willingness to obtain)
- Current Australian driver's licence and reliable vehicle (preferred)
- Right to work in Australia
- Strong communication and interpersonal skills
- Genuine passion for supporting people with disability
- Ability to work independently and as part of a team
